Cancer Screening
The Huronia Nurse Practitioner-Led Clinic is part of a Cancer Care Ontario initiative to provide cancer screening for clients and non-clients between the ages of 21-65. Please call our admin team at 705-835-7545 to book an appointment, or if you have any questions about whether you qualify.
Diabetes Program
The Huronia Nurse Practitioner-Led Clinic Diabetes Program is a multidisciplinary team approach to helping patients with diabetes better manage their disease. Our goal is to improve health outcomes for adults living with Type 1 and Type 2 diabetes, by providing education that supports individual learning and patient self-management.
While in this program you will be seen by diabetes educators including a Pharmacist, a Registered Nurse and a Social Worker. Each team member has an important role in working with you to keep you well. Our team is here to help you set your own goals and provide you with education and coaching regarding your diabetes. We will provide both individual and group appointments to help you understand and manage your diabetes. This comprehensive program is available to all clients of Huronia Nurse Practitioner-Led Clinic. Clients are referred to the Diabetes Program by their Nurse Practitioner.
Chronic Disease Management Program

Through a team-based approach to care, the Chronic Disease Management Program provides a variety of services for clients living with chronic health conditions such as diabetes, high blood pressure, congestive heart failure, and arthritis. Our goal is to assist you in staying healthy and active through support from our clinic as well as connecting you to community based services and programs to further support individuals and families.
